New Patients
Your First Visit
Before your first visit at London Heritage Dental, we will ask you to share background information about your general health, medications, previous surgeries, dental procedures and lifestyle. This information is important to us as many different aspects of the body are connected to your oral health.
At the time of your consultation, we ask you to provide:
• Any x-rays if applicable or the name of your previous dental office.
• A list of medications you are currently taking (this can be obtained from your pharmacist).
• If you have medical or dental insurance, please bring the necessary completed Patient Registration forms. This will allow us to start processing your claims right away.
Your initial appointment will consist of a consultation. Please note that all patients under the age of 18 must be accompanied by a parent or guardian at the initial consultation visit. If treatment is necessary, a treatment plan can be discussed right away. More complex cases may require a second appointment.

Payment Policy
Payment is due at the time of service. London Heritage Dental accepts MasterCard, Visa, Debit, Cash, and Cheques. We can also direct-bill to most insurance companies upon request.
Having dental insurance is a reason to smile, but don’t let it limit the care you receive. Most insurance policies will provide partial or complete reimbursement for your oral health needs, but at times, your care may exceed your insurance coverage. Dr. Lekhi can work with you to select treatment options based on what is in your best interest to optimize your dental health.
No one likes a surprise when it is time to pay, which is why we will explain all treatments and associated costs before proceeding with any services. In addition, we can send pre-determinations to your insurance company before we conduct a treatment. We want to ensure that you are always fully informed before consenting to any procedures.
Cancellation Policy
While we understand that emergencies occur and schedules may change, we ask that whenever possible, patients provide two business days’ notice for a change in appointment times. If this policy is not followed, a $75.00 fee may be applied.
X-Rays
If your dentist or physician has taken x-rays in the past, you may request that they forward them to our office. If additional x-rays are necessary, we are happy to take them at our facility.
